IL Medical Cannabis Application Update

As of December 1, the Division of Medical Cannabis will no longer accept paper applications for the Medical Cannabis Patient Program.

Any paper applications or fees mailed after November 30 will be returned with instructions for online submission. The Division of Medical Cannabis will be rolling out a new registry system in 2021, which will be entirely online. All patients who hold active registry cards will be receive more information before the new system is put in place.

Certain paper forms are still accepted by mail.

  • Caregivers Applications: Caregiver applications will still need to submitted via paper application if the caregiver is applying after the patient has a registry card or provisional access.

  • Lost/Stolen Replacement Forms

  • Change of Information Forms

  • Waiver to Increase Allotment: only if a patient does not submit this with the initial application.
