Important information for your first visit to the dispensary:

To complete the new patient registration process, fill out the above form and bring it in on your first visit. If you are unable to bring in a completed form, they will be available at the reception desk.

At your first visit, you must present the form of identification used during the Board of Pharmacy registration with your physician (driver’s license, passport, state ID,etc.) Your ID must be in its original form; no copies accepted. 

You must also have your medical marijuana card number readily available to complete registration. A digital copy of your card is accepted.

If you are a caregiver completing new patient registration WITHOUT the patient present, you must present the patient’s form of ID used during the Board of Pharmacy registration with your physician.
